The 2009 People Helping People (PHP) Awards Nomination Form

PRESENTING THE 2009 PEOPLE HELPING PEOPLE AWARDS CALL FOR NOMINATIONS

BE RECOGNIZED FOR YOUR CONTRIBUTIONS TO THE COMMUNITY
A Sacramento Tradition—doing good is part of doing business. Sacramentans consistently donate their resources, time and talent to local organizations and activities. A passion for service permeates our community. Too often these good works go unrecognized, and the People Helping People Awards aim to change that.

In the Business category you can nominate your own company or executive leader. The Community Services Planning Council encourages you to nominate your own business or executive leader for a prestigious People Helping People Business Award. Who knows better than you what your company is doing for the community? Besides, you deserve the recognition for your good deeds!

Awards Selection. A panel of former Award recipients and community and business leaders will select a winner in each of the award categories.

Nominee Recognition and Awards Presentations. Award winners will be announced at the 21st Annual People Helping People Awards on Wednesday, April 24, 2009 at the Radisson Hotel Sacramento. All nominees will also be recognized at the event. The luncheon is one of Sacramento's premiere annual events, attended by more than 600 business and community leaders.

NOMINATION INSTRUCTIONS
You may submit your nomination anytime but no later than 5:00 p.m. on Friday, January 23, 2009. Please do not send any other supporting materials, as we cannot make copies of them for the awards review panel.
