Community Services Directory Requirements

The Community Services Directory lists nonprofit and governmental agencies, organizations and associations that provide community service programs in the Sacramento, California community.  Agency and programs must meet the Inclusion/Exclusion criteria to join the directory database.

Agencies who meet the Inclusion/Exclusion criteria can complete and submit the on-line form (or complete the printable form and send it to us by FAX, US mail or in person). Provide complete information as you wish it to appear in the directory. Do not include any information you do not want published.

Non-Profit and government contracted agencies must provide a copy of their 501(c)(3) tax exemption status letter with their application.  Government agencies must provide a Federal Employment ID Number (EIN) to apply.  A copy of a 501(c)(3) tax exemption status may be submitted with the printable application in place of the EIN.  Application forms can be sent by mail, fax, or brought in-person to the Community Services Planning Council office at the address listed below.
